The goal is to collect as many pounds of cereal to provide breakfast to underserved children in the area. Improve Average Length of Stay - Congestive Heart Failure

In efforts to meet the quadruple aim, we work to decrease and maintain a low average length (ALOS) of stay which allows our patients the ability to return back to their normal lives, as much as possible. In 2018, we were able to significantly improve our Congestive Heart Failure ALOS from 0.75 to 0.51.

- Daily interdisciplinary team meeting ensuring all stakeholders are aware of the plan, any pending orders/needs and a general update on patient progress.
- Early identification of home needs that may delay length of stay.
