Helping Patients Find the Right Doctor

Our top priority is our patients and the providers who care for them. To deliver high-quality care in a safe environment, we offer tools to help patients and providers connect. One of these tools is the Physician Referral Service Program, which helps patients find the care they need.

What Is the Physician Referral Service Program?

This program helps patients find doctors who:

  • Provide quality care
  • Are easy to access
  • Focus on great customer service

It powers the Find a Doctor section on our hospital websites, showing randomized search results based on what patients are looking for.

This also helps our patients in our ER and Urgent Care Follow-Up Programs, as the providers discharging patients will know which providers have attested to accepting new patients in their clinical practice.

How Providers Can Get Connected

Patients often search online when choosing a doctor. Providers in this program are presented to patients through our “Find-a-Doctor” website tool, follow-up services and our patient contact center. You may be eligible to be included in our hospital referral service program, depending on your specialty and medical staff category. There’s no cost to join.

Who Can Join the Program?

To participate, providers must:

  • Have specific medical staff privileges at our facility
  • Accept new patients in an outpatient setting
  • Be in good standing with the facility
  • Be credentialed for the services listed in their profile

Note: Depending on the hospital program you would like to join, your employment or financial interest in a competing health system may affect your eligibility.

 How it helps patient care

Designed for patients who are treated in an emergency room or urgent care center and discharged home.


Patient care liaisons are available by phone to help with scheduling the patient’s specialty follow up appointment as recommended by ER or urgent care provider. Patients may also receive text messages to help facilitate follow up appointment scheduling.

Offered in most facilities, the point of care scheduling program provides onsite assistance to patients in need of specialty follow up appointment as recommended by ER provider at time of discharge. The platform utilized by our team allows direct communication with the physician’s office to optimize continuity of care and to support the patient’s transition of care experience. HIPAA-compliant platform that allows for secure communication, image exchange, and workflow management among healthcare providers.  

Created to offer continued support for patients following hospital visits.

Led by clinically trained nurse navigators, Care Navigation helps patients with follow- up appointment services and/or answering questions regarding their care within the following specialties: cardiology, orthopedics, oncology, and high-risk perinatal are.

Cancer Navigation provides patients and their families with support through every step of the treatment spectrum, from prevention to palliative and support care.
 How to participate  Physicians must meet criteria and be included in our hospital referral service program. Physicians must create an account with the platform vendor and agree to participation terms in order to receive notifications for appointment requests. Physicians work directly with the vendor to purchase an account in order to collaborate with our physicians and clinic staff. Physicians must meet criteria and be included in our hospital referral service program.
