Our approach to heart transplantation is a team approach that includes world-class physicians, transplant nurse coordinators, social workers, dietitians, financial coordinators, administrative staff and support staff.
Transplant Nurse Coordinators
Registered nurses with expertise in transplantation, ventricular assist devices, and pulmonary hypertension specially trained to care and provide support for patients and their families before and after transplantation. These coordinators serve as the patient's main contact for questions from the time of their evaluation throughout the entire transplant process.
Available for evaluation and supportive counseling for patients and their families throughout the phases of transplant. If information is needed regarding local housing, fundraising and other transplant resources, the transplant social worker is the key contact. Social Workers also assist with psychosocial evaluations, initial insurance evaluations, and assistance with medications.
Evaluates patients nutritional status before transplant, and provides education and assistance with nutritional needs before and after the transplant.
Helps patients understand the cost of transplantation, their individual transplant benefits and the overall transplant financial process that is required for all patients. The financial coordinator is available to answer questions about insurance coverage and to assist patients in identifying financial resources.
Administrative and Support Staff
Schedules evaluation, follow-up appointments, surgeries, and ensures all appropriate forms needed before and after transplantation are successfully completed.
